Beyond Pores and Acne Scars: What Our New ELLISYS SENSE Study Reveals About the Eye Area
I am pleased to share that our new clinical paper on periorbital wrinkle improvement with ELLISYS SENSE was published online on July 17, 2026, in the international journal Lasers in Medical Science.
The article, titled “Dual-depth 25-pin insulated microneedle radiofrequency for moderate-to-severe periorbital wrinkles,” lists me as a co-author and Éclat de Cheongdam Clinic as one of the participating institutions.
For many patients and physicians, microneedle radiofrequency is most familiar as a treatment for enlarged pores, uneven texture, and acne scars. This study is meaningful because it evaluates a broader clinical application: the delicate skin around the eyes.
Why the Eye Area Requires a Different Approach
Periorbital aging is not caused by one factor alone. The eyelid skin is naturally thin, while repeated blinking, orbicularis oculi muscle movement, photoaging, and progressive changes in collagen and elastic fibers may contribute to fine lines, static wrinkles, laxity, and a tired appearance. For this reason, treatments aimed only at muscle movement or superficial texture may not fully address structural changes within the skin.
The protocol evaluated in this study used a 25-pin insulated microneedle tip, selected for the limited surface area and thin skin of the periorbital region. The insulated structure was designed to concentrate RF energy near the exposed needle tips while reducing unnecessary heat transfer along the needle shafts.
Vacuum-assisted fixation was used to stabilize the mobile, curved tissue around the eyes. A duty-controlled Double Shot mechanism then delivered RF energy sequentially at 1.5 mm and 1.0 mm during a single needle-insertion cycle. This approach allowed two dermal depths to be treated without repeating the full mechanical insertion process.
What the Study Evaluated
The observational study included 32 adults aged 30 to 60 years with moderate-to-severe periorbital wrinkles. Participants received three treatments at four-week intervals and were evaluated through week 24.
The analysis included blinded clinical wrinkle grading, three-dimensional measurements of wrinkle depth, skin-elasticity testing, patient satisfaction, treatment discomfort, recovery time, and safety monitoring.
At week 24, the mean Fitzpatrick Wrinkle Assessment Scale score had decreased from 5.44 to 3.38. Average and maximum wrinkle depth also decreased, while the measured skin-elasticity parameters improved progressively throughout follow-up.
Patient satisfaction was also encouraging: 90.63% of participants reported being satisfied or very satisfied at the final follow-up. The mean bilateral procedure time was approximately 7.2 minutes, the mean pain score was 3.3 out of 10, and the average clinical downtime was approximately 1.5 days. Reported reactions were primarily temporary mild redness and swelling.
From Early Clinical Experience to Academic Evidence
I was the first clinician in Korea to begin using ELLISYS SENSE and also the first to introduce its clinical applications through an academic presentation. Since then, I have continued to share treatment techniques and practical experience with physicians from Korea and abroad.
Being an early user of a medical device carries a responsibility beyond simply adopting new technology. It requires careful observation of depth, energy, tissue response, anatomical differences, recovery patterns, and safety. It also requires that accumulated clinical experience be shared in a form that other physicians can examine and reproduce.
For me, this publication represents an important step in that process. Experience from daily clinical practice has now been organized into objective measurements and presented through an internationally reviewed scientific journal.
What This Publication Means
This study does not suggest that one device can replace every established treatment for eye-area aging. Surgical correction may remain more appropriate for pronounced eyelid ptosis or substantial skin redundancy.
Rather, the findings support dual-depth insulated microneedle RF as a minimally invasive option for selected patients with periorbital wrinkles and reduced skin elasticity who prefer a short procedure and limited recovery burden.
As an observational study with a relatively small cohort, the results should also be interpreted with appropriate academic caution. Larger controlled studies and longer follow-up periods will be valuable in refining treatment parameters and determining long-term maintenance strategies.
Nevertheless, this publication expands the clinical conversation around ELLISYS SENSE. Beyond pores and acne scars, its carefully controlled energy delivery may also have a role in the precise rejuvenation of delicate anatomical areas.
